Important Information
Thank you for your interest in participating in this soccer tournament! Just a few things to consider before you complete the form:
- All team members must be between 18-39 years of age (ID’s will be checked at the gate with roster).
- Teams must be co-ed: each team must have a minimum of 4 females/4 males.
- Only ONE team per parish will be accepted.
- While only 7 players are mandatory, you are strongly encouraged to fill all 14 team spots.
- In order to successfully submit this form, please have these details ready in advance for each team player: first and last name, age, birthdate, sex, phone number, emergency contact name, emergency contact phone number.
- If there are changes to the team, please send an updated roster to Brilema Perez (bperez@sdcatholic.org) by July 16.
- Each game will be 30 minutes long with 7 vs. 7 (including the goalie).
- Only 16 teams will be allowed to register and priority will be given on a first come first serve basis.
- No additional teams will be added on the day of the tournament.
- Registration Deadline: Friday, July 11th, 2025, or when the 16th team registers.
- The team captain will receive the schedule and rules within a week of registration.
